You may click the button below to fill out Form DS-156 online. You must then print the resulting completed document, including the barcode on the last page which contains all of the information you have entered. This will help the U.S. Consulate to process your application more quickly.

Electronic Visa Application Forms

Form DS-156 can be filled out on-line to assist in the processing of your application. If you have not updated your browser in a long time then it is possible that you may encounter some difficulty using the online e-form. Most users will have no difficulty.

If your form submission results in an error message then please try again later. Once in a while, the server becomes overwhelmed by there being too many users at the same time.

Important Notice to All Non-Immigrant Visa Applicants
Please Read Carefully

Effective November 1, 2006:

All visa applicants, wherever they apply, are now required to submit electronically-completed DS-156 application forms.
Hand-written forms will no longer be accepted.

If you appear for your interview with a hand-written application you will be turned away and assigned another date for your interview.

You MUST complete the electronic form on-line (all 3 pages). Print out the entire form, including the third (barcode) page, and bring it with you to the interview.
